Standards and Guidelines

**For non-adults (those less than 18 years old or 18 years old and still in secondary school), please have you and your parent/guardian electronically sign below. There will be a second option to add your parent/guardian electronic signature.

Please fill out the following form stating you have read and acknowledged the Standards of Behavior and Boundaries document and agree to the following statement:

I hereby certify that I have received, read and understand the Diocese of Toledo's Standards of Behavior and Boundaries for all Priests, Deacons, Religious, Pastoral Ministers, Principals, Teachers Employees, and Volunteers in the Roman Catholic Diocese of Toledo in America (Standards) and Guidelines for the Prevention of Abuse of Vulnerable Adults (Guidelines). I agree to abide by the policies contained herein. If I am a priest or other cleric, and violate these policies, I understand that the Diocese of Toledo will take disciplinary action in its sole voluntary discretion, up to and including termination of employment or volunteering. In all cases involving suspected abuse of a minor, I understand that law enforcement authorities will be promptly notified.
